Posted here are our Registration Form with Medical Release and Concussion Information. Feel free to download and complete prior to your arrival at our registration site. We will also have forms available at the registration site and will need to verify child's age via birth certificate.
Registration will only occur at our registration site or via mail. All mail-in registrations must be post-marked no later than 30 January in order to guarantee a spot on a team.
